Background: Information about the relationships between physical fitness, body composition and nutrition has increased in recent years; however, little is known about physical fitness and the coexistence of under-/overnutrition among children living in disadvantaged areas. Objectives: To determine the physical fitness status and its association with body composition, growth and selected socio-demographics in primary schoolchildren from disadvantaged communities in the Nelson Mandela Bay region. Methods: Nine hundred and sixty-five children (49% girls, M=9.5 years) participated in this cross-sectional study. Height and weight were measured to establish body mass index, and height-for-age z-scores. Physical fitness was assessed using tests from the Eurofit Physical Fitness test battery (flexibility, upper/lower body muscular strength and cardiorespiratory fitness). Between-group differences and cross-sectional associations were examined with univariate (Chi2-tests, analyses of variance) and multivariate methods (mixed linear/logistic regression). Results: Most children had normal weight (76.7%), while 4.5% were underweight and 18.7% were overweight/obese. Underweight children and children with stunted growth (11.5%) had lower average upper body strength (p<0.001). Overweight/obese children had lower scores in weight-bearing activities (p<0.001). Children with higher socio-economic status were more likely to be overweight and obese (p<0.001). In the multivariate analyses, sex, age, body mass index, and stunting were associated with children's physical fitness. Conclusion: Fitness assessments seem to be a relevant measure of the current health status of children in disadvantaged settings. Compared to international norms, the children in this study had relatively low scores for both upper- and lower body muscular strength. Therefore, effective school-based intervention programmes should be developed to improve children's physical fitness in disadvantaged schools.

RESUMO

This study provides a comprehensive estimate of the economic burden of mental health problems in Canada in 1998. In particular, it estimates the cost of non-medical services that have not been previously published and the value of short-term disability associated with mental health problems that were previously underestimated according to the definitions used here. The costs of consultations with psychologists and social workers not covered by public health insurance was $278 million, while the value of reduced productivity associated with depression and distress over the short term was $6 billion. Several data limitations suggest that these are underestimates. The estimated total burden of $14.4 billion places mental health problems among the costliest conditions in Canada.

Rat collecting ducts exhibit type I or type III K(+)-ATPase activities when animals are fed a normal (NK) or a K(+)-depleted diet (LK). This study aimed at determining functionally the cell origin of these two K(+)-ATPases. For this purpose, we searched for an effect on K(+)-ATPases of hormones that trigger cAMP production in a cell-specific fashion. The effects of 1-deamino-8-D-arginine vasopressin (dD-AVP), calcitonin, and isoproterenol in principal cells, alpha-intercalated cells, and beta-intercalated cells of cortical collecting duct (CCD), respectively, and of dD-AVP and glucagon in principal and alpha-intercalated cells of outer medullary collecting duct (OMCD), respectively, were examined. In CCDs, K(+)-ATPase was stimulated by calcitonin and isoproterenol in NK rats (type I K(+)-ATPase) and by dD-AVP in LK rats (type III K(+)-ATPase). In OMCDs, dD-AVP and glucagon stimulated type III but not type I K(+)-ATPase. These hormone effects were mimicked by the cAMP-permeant analog dibutyryl-cAMP. In conclusion, in NK rats, cAMP stimulates type I K(+)-ATPase activity in alpha- and beta-intercalated CCD cells, whereas in LK rats it stimulates type III K(+)-ATPase in principal cells of both CCD and OMCD and in OMCD intercalated cells.

This study examines eight measures of mental health and looks for associations with nine potential demographic and psychosocial determinants. Data from the National Population Health Survey (NPHS), analyzed by logistic regression, reveal consistently strong, graded, independent associations of current stress, social support, life events, education and childhood traumas with both positive and negative indicators of mental health status. Sex differences exist for four of eight measures. For most indicators, mental health is relatively poor among youth and improves with age. Physical and mental health problems are associated. There is no independent relation between mental health and income adequacy or province of residence. Two measures used in several previous Canadian surveys are recommended for inclusion in the NPHS to better monitor population mental health.

Kidneys are the main effectors of the maintenance of potassium balance, under both normal and altered conditions of dietary potassium uptake. The collecting duct system plays a major role in this control of potassium homeostasis because most of the filtered potassium is reabsorbed between the glomerulus and the distal convoluted tubule: under normal physiological conditions or in response to potassium loading, collecting ducts adjust their rate of secretion of potassium into urine so as it matches the dietary daily intake, whereas in response to restriction of potassium ingestion, this secretion process is mostly curtailed, and a reabsorptive mechanism appears. In this short review, we analyzed the cellular and molecular mechanisms underlying transepithelial transport of potassium in the collecting duct and their adaptation in response to potassium depletion. A special emphasis is given on the axial and cellular heterogeneity of the collecting duct with regard to potassium transport and its adaptation. We also discuss the factors controlling duct hypertrophy and hyperplasia during potassium depletion and their possible relationship with the control of potassium conservation.
